This Website Privacy Policy describes how Rogue Holistic Therapy (“we,” “us,” or “our”) collects, uses, and
protects information gathered through rogueholistictherapy.com (the “Site”). This Policy addresses information
collected through your use of the Site itself. It is separate from, and does not replace, our HIPAA Notice of Privacy
Practices, which describes how we use and disclose Protected Health Information (“PHI”) in connection with your
care. Where this Policy and the Notice of Privacy Practices both apply, the Notice of Privacy Practices governs the
handling of PHI.
Depending on how you use the Site, we may collect:
Please do not include diagnoses, treatment history, symptoms, or other clinical detail in the Contact Us form or the
scheduling tool. Neither is monitored on an emergency or continuous basis, and neither is an appropriate channel for
urgent or clinical communication. If you are an existing client, please use the secure client portal or another method
your Provider has approved.
We use information collected through the Site to respond to inquiries, schedule consultations and appointments,
determine whether our services may be a good fit for you, maintain and secure the Site, and comply with applicable
law.
Contact Us form submissions are delivered to admin@rogueholistictherapy.com, an email account hosted on Google
Workspace and covered under a signed Business Associate Agreement. Access to this inbox is limited to designated
practice staff.
We use third-party service providers to operate the Site and deliver certain functions, including website hosting,
scheduling (Google Meet), and our client portal (Therapy Notes). We share only the information reasonably
necessary for these providers to perform their functions. Where a vendor creates, receives, maintains, or transmits
PHI on our behalf, we require a signed Business Associate Agreement.
The Site may use essential cookies and limited analytics technologies to understand site traffic and improve
functionality. You may disable cookies through your browser, though some features of the Site may not function
correctly as a result.
The Site links to third-party tools and websites, including our client portal at Therapy Notes and individual Provider
scheduling calendars. These third parties operate under their own privacy practices, which we do not control. We are
not responsible for the content or privacy practices of any linked site.
If you provide a phone number and consent to text communications, we may send appointment, scheduling, and
related service messages. Message and data rates may apply. You may opt out at any time by contacting us at
admin@rogueholistictherapy.com; opting out does not affect your ability to receive services but may require an
alternate contact method.
Email and text messages are not guaranteed to be secure. We use reasonable safeguards, but electronic
communication carries inherent privacy risk. This Policy does not authorize sending detailed clinical information by
unsecured email or text.
We use reasonable administrative, physical, and technical safeguards designed to protect information collected
through the Site. No website or email system can be guaranteed completely secure. We retain website information
only as long as reasonably necessary for the purposes described in this Policy or as required by applicable law.
You may ask us to update or delete non-clinical contact information, or to change your communication preferences,
by contacting us at admin@rogueholistictherapy.com. This does not apply to information we are required to retain
for clinical, legal, billing, or professional purposes.
This Site is not directed exclusively to adults. Consistent with Oregon law (ORS 109.675), individuals as young as
14 may independently submit the Contact Us form or request a consultation to seek mental health treatment without
parental consent. A parent or guardian may also submit these forms on behalf of a minor. Information submitted
through the Site by a minor is handled under the same protections described in this Policy. How parental
involvement, confidentiality, and access to records are handled once treatment begins is governed by the practice’s
Informed Consent for Treatment & Services Agreement and Practice Policies, not by this Website Privacy Policy.
If you believe your privacy has been violated, you may contact us using the information below. Complaints
involving Protected Health Information should be directed as described in our Notice of Privacy Practices, which
also describes your right to file a complaint with the U.S. Department of Health and Human Services, Office for
Civil Rights.
We may update this Policy from time to time. The current version will be posted on the Site. Material changes
affecting the handling of PHI will be addressed through an updated Notice of Privacy Practices in accordance with
HIPAA’s requirements.
